I am working in a non-profit org. which needs to have forms on the website also available in iPhones, iPads, WinMo, Blackberry or Android devices. Those forms should be able to (if not) take a current picture of yourself. The point is I'm thinking about PDF forms, or should they be the ones that I'll use online? All completed forms will be sent back to us and stored in a database. You may ask, why PDF? The reason behind the PDF form is for offline sending of the same information, as not every place our organization works has WiFi or internet - so the idea is that something like having those PDF forms printed, completed and scanned back (via OCR or other viable data-copying/form extracting solution) and still being saved back to the database.
