I have used Help & Manual for several years: http://www.ec-software.com/[^]. It is the best one I have ever used. Highly recommended.

I will second that. We evaluated and and used a few others before finally sticking to help and manual.

We had some issues with HelpMaker (mostly related to formatting muck-ups and the inability to edit the HTML for each page). As a result we're now trying WinCHM[^], which seems to be altogether more suitable (for us at least).
