Loose
It's "lose" not "loose". :) This is fast becoming the most common spelling mistake online.

I'm still waiting... unless she decided to personally deliver a reply... any news of any axe "accidents" since the original post?

Amen! If I had a nickel for every time I've seen that over the last few years, I might not "loose" it, like I feel more and more like doing the more I see people mangle "loose" and "lose". It drives me CRAZY!

I'm puzzled as to why the bag didn't have a tether attached to it.

because instead of drifting off harmlessly due to a lateral acceleration; the tether would cause it to swing, wrap around something, and smash into it, potentially doing serious damage if it lead with the pointy end of a tool.

How about a tether that retracted and held it tight against the hull of space station?

of course a similar thing happened during the original hubble fix. It is the fact that the suits are ungainly and movement difficult. You can train a person only so far, then reality must force that person to adapt. Those suits don't offer enough movement to adapt. Thus we lost a special made wrench on the original hubble repair.
