Besides "Fine, Thank you " Is there anyother answer?

"I am well, how are you" is the correct response. Should they ask "How do you do?", the correct response is to say "How do you do?" back.

I cannot cope with inane small talk so usually go with something like the following; Fair to middlin' Splendid Reasonable Moist Adequate and so on. My usual response to being greeted with "Morning" when get to the office is "Yes, yes it is". To be honest I irritate myself most days.

Yes, but we actually say "How d'y'do". This only applies to the English BTW, it confuses the hell out of the Americans. USians assume that we are just like them (after all the US was founded by the British for the most part and we sort of have the same language). Then they fall foul of the (often highly complex) social norms that we are all used to when they come to the UK. To a non-european (and to many European) UKian culture is probably every bit a odd as Japanese culture is to us. This book Watching The English[^] helped my wife explain a lot of my behaviour. The worst thing is that in the UK we think that we are normal, and everyone else is odd an inexplicable (comes from living on an island I suppose) :-).

Says who? Mrs Dullpedant book of English Elocution published in 1932? Bollocks. The correct response is anything you damn well like and commonly is 'mustnt grumble', or, 'OK', or, 'not too bad'. Follwed by 'and you'? 'hows it hanging' is a more common greeting aswell, or 'hows it going' or 'hows life' or 'what do ya know' etc etc etc.

The second "How do you do?" acts like a null on a null terminate string, that's why the rest of the conversation is so tense, formally it has stopped :-)
