News report here[^] Mr Sherman, the special agent in-charge in San Diego, said the event is not indicative of the high standards to which he holds his employees. He said he has personally ordered an extensive review of his office's policies and procedures. Fifty years ago, they would have found the person responsible and kicked his ass. Now, they simply change the software. The world is gradually being dumbed down. Idiocracy[^] is not merely a funny movie, it's a prophecy.

I did not read the report that you linked to. However, based on the rest of your post, I would agree. Using the word "investigation" and/or "review" is an all to commonly used term to aid in pushing the matter under the carpet.
