I have read that version 4.5 of the .NET framework actually replaces version 4.0, implying that version 4.0 no longer exists on the machine after upgrading to 4.5. What does this mean for running Visual Studio 2010 and 2012 side by side? Is anyone doing this right now? Are there any issues?

The difficult we do right away... ...the impossible takes slightly longer.

As far as I am aware the latest version of .NET includes all previous versions for backwards compatibility.

One of these days I'm going to think of a really clever signature.

Yes, when I installed the 2012 RC, it screwed up my install of 2010. I had to repair the install. Grrr. Same thing happened when I installed the RTM (double dutch grr), but that may be because I had previously installed the RC on the same machine. Other than that, they both run well side by side.

I installed 2012 Professional on a desktop that already had 2010 Professional on it and I didn't get any issues at all. Not sure if the issue you may have encountered was due to the 2012 was a RC version or if they already fixed that.
