Lots of those pictures are hysterical. But they're funny because you know that know lives were lost. The pictures of the Concord do not, however, fall into what I would consider to be an "oops". Marc VS2005 Tips & Tricks -- contributions welcome!

The oops is that despite not carrying periodic checks and someone calling the control tower 30 minutes before they still didn't remove the debris from the runway that punctured the wing tank :mad: The tigress is here :-D

The folks at holloman actually sent me the hair today gone tomorrow image first. My bosses brought it to me second... and my peers brought it to me 3rd. What is it about having the longest hair in the building that means you get these warnings? hmmmm Actually it was a sad event, and not very smart also. I generally tie my hair back (I think a few people here, though I don't recall who, commented about the preference of letting the hair free now you know why). I don't work near machines often, when I do I tuck my hair in the back of my shirt (still tied). We recently had another accident, it was said that several warnings/questions were made because it just didn't "look safe". Our boss commented that many accidents happen this way, someone thinks it isn't safe, and someone else reassures them that it is truly safe... then the accident happens. I wish I had known of this site earlier, I think I found something on the drone Qf4 nosedive a couple years back last week.
