    The Columbia Accident Investigation Report[^] has been released. From what I can gather it looks like the lessons from Challenger were not learnt and that NASA are trying to do too much with too little. The problem of foam damaging the shuttle was long known about. I guess the balance between cost and safety wasn't quite right. Derek Lakin. I remember a while ago reading that the shuttle was originally designed to beat a 1 in a hundred odds of mission catastrophe. ( dunno how true that is ). Regardz Colin J Davies

        Even everyday things like subways aren't 100% safe and when you're pushing the envelope, you can expect problems. I know that for the Apollo program, they expected to lose one crew and they did although it was on the ground. I worked at Rockwell while the shuttle was being designed. Although I was not in Space Division, I got to visit there while taking a finite element course from one of the engineers who was working on the ceramic tiles. The insider term for the tiles was "mummy dust" as they were very fragile.
