I don't remember the last time I saw one of these here. When it's hiding in plain sight, there has to be a little misdirection. :)

Weight? To regain it, I ate dementedly once the bar
    ^^ ^^ ^ ^^^ ^
    opened (def)

    INITIATED

I think @Rich-Leyshon had it but just didn't see the definition, so I'm inclined to give it to him, especially as I don't recall him setting one. Want to take it tomorrow, Rich?

Yes, that's what I had but didn't understand why! I am losing track of days but isn't tomorrow Saturday? Alas, Monday morning I have a hospital appointment so won't be able to (that and the fact that the prospect of doing my first one terrifies the hell out of me!)

My bad, apparently. I thought you were referring to a colonoscopy.

Ah right, no. I have watched one though in the past! This is the marginally prettier end - Maxillofacial.

The rest of it (by my understanding) doesn't matter when the word appears as consecutive letters within the clue, so long as the definition is still at either end.

In looking into it further, I learned that there should nonetheless have been an indicator that hinted at the word being embedded within the clue, which was therefore unfair.

Part regain it I ate dementedly opened (9) - would be a more traditional clue

It doesn't, other than to compose something that makes sense.
