That's one of the reasons skydivers use square canopies nowadays, they open a lot more cleanly and have a lower failure rate.

If you read the articles published elsewhere, it wasn't the actual Orion capsule chutes that failed, it was the "staging" chutes being used to "set up" the test itself by getting the capsule properly aligned to simulate reentry attitude. Sadly the impact on program schedule is the same as if the re-entry package itself had failed...
