Loo,k at the data and decide how many items you want in each row. You then can set the values for your loop. For each iteration of the loop: 1. Emit a <tr> element. 2. Emit a <td> followed by the first data item, followed by </td> 3. Repeat number 2 for each remaining data item. 4. Emit a </tr> item for the end of the row.

That message most likely means that JDBC cannot connect through port 3306, the mysql driver is not listening there, or some part of that path is not valid.

Yes you are on the correct track with this info. If you are importing directly from the CSV then the file format MUST match the database fields. You could write a parser which does the importing for you, this would give you an opportunity to trap crappy data before it hits your DB. CSV has some gotchas, if it is not well formed, dealing with commas in the data, then you are in big trouble. "Form" implies a standardised format otherwise it is just free form text.
